AWARD NOMINATION/SUBMISSION FORM

I understand the Award testimonial I provided (hereinafter called “Award Testimonial”), including all text, images, and video provided, may be used in promoting Virginia Police Work Dog Members and their Accomplishments.

I authorize Virginia Police Work Dog, along with its Board Members and representatives, to use The Award Testimonial in promotional efforts, including but not limited to websites, advertisements, social media, brochures, and event materials.

I waive any right to inspect, edit, or approve materials where The Testimonial is used. I also waive any claim to ownership of The Testimonial or related materials and its use.

Virginia Police Work Dog may make reasonable edits to The Testimonial for clarity, length, or formatting purposes, while maintaining the original meaning. I agree that my comments may be published as written.
